Patient Portal

Once you call and schedule your appointment, you will receive an email to activate your patient portal. You must complete your information there prior to your appointment. Once you activate your patient portal, you can always come back to the website and use the link below to access the portal where you will use your username and password to access your information.

However, you may be required to obtain a referral from a primary care physician prior to scheduling an appointment. Make sure contact your insurance provider to learn about your coverage.
